Our Mission
Galen Care Partner’s is a case management provider for Chronic Kidney Disease and End Stage Renal Disease. Our primary goal is to maximize care and wellbeing, while at the same time minimizing patient, providers, and guarantors out of pocket costs and hospitalizations. Using our knowledge and a variety of services that we provide our cliental, we will help to make their quality of life better for them and their family.
Kidney Education
Kidney damage can often be stopped, or at least slowed, with medications and lifestyle changes. We encourage you to read the patient education materials here and share them with your family members and caregivers. As questions arise, we look forward to answering them.

Kidney transplantation can be a difficult transition for you and you your family. Galen Care Partners will assist you through the process. We have a specialized team that will coordinate, schedule, facilitate, and answer your questions and concerns that you and your loved ones may have.
